There are also five ways to complete the Consent forms:
- Sending the attached form back to me via email signed (april.waterford@beechbrook.org) OR
- Sending me a email stating parent name, student name, and Yes, I consent to their participation in group (april.waterford@beechbrook.org) OR
- Giving me a call/leaving a voicemail with parent name, student name, and Yes, I consent to their participation in group (216.203.2524) OR
- Sending me a text message with parent name, student name, and Yes, I consent to their participation in group (216.203.2524) OR
- Finally if a kid wants to hop in the group at the last minute on Friday without having used one of the above options their parent can hop on the screen for a second at the group to give a quick verbal consent.
